1. Download the Digital Excavation Form and fill it out.
2. Submit your completed Digital Excavation Form
Email your completed form requests to
HT-Excavations@hawaiiantel.com.
Attach your signed Hawaiian Telcom Excavation Permit and plans or map of
the location.
